Funding Opportunity for Illinois – We Choose Health Initiative

Grant: We Choose Health is a multi-year effort to encourage and to support population-based obesity and tobacco prevention in Illinois communities. We Choose Health is a sub-award program to fund organizations and coalitions working on chronic disease prevention at the local level.

Funder: Illinois Department of Public Health

Eligibility: Local government entities, schools systems, community organizations, and coalitions ready to take on this important work for the health of Illnoise and its residents are eligible to apply. Please note that due to restrictions placed on the Illinois Department of Public Health by the CDC, this grant cannot fund communities in the five largest counties of Illinois (Cooke, DuPage, Lake, Will or Kane Counties).

Amount: $3.3 million dollars, with maximum awards of up to $300,000 per year for a period of four years.

The We Choose Health initiative offers applicants the opportunity to mix and match among a variety of strategies covering these target areas.

To apply for this funding, you must submit a completed letter of intent form via e-mail by 5pm, May 18, 2012. Completed applications are due by e-mail no later than 5pm, June 15, 2012.
